Most medical insurance, including Medicare, Medicare Supplement and Medicare Advantage in the U.S. provide very limited or no coverage outside the U.S.
Below are links to webpages that provide Travel Medical and Travel Trip Protection. I do not leave the U.S. without buying a Seven Corners medical plan. I find them very reasonable and although I have not had to use the coverage, having the peace of mind and also the advice that comes from Seven Corners should a problem arise, is a great comfort.
